We recognise the important role data and technology play in achieving mana whānau, whānau ora. Mōhio is our innovative data and digital ecosystem. As a single system, Mōhio links patient experiences with analytics, creating actionable intelligence for decision makers. Analytics are offered in tabular machine-readable data and via dashboard, or as required by clients. Feedback from users tell us Mōhio technology dramatically improves the way they do business and helps drive improved patient outcomes and access to entitlements.

Mōhio endorses prioritisation of Māori and other high priority whānau, enables consistency across practices in claiming, reporting and patient care and provides real time feedback on performance including performance against National Health Targets at practice and PHO level.

National Hauora Coalition is proud to have developed our products in-house. Within the team are data analysts, data scientists, developers and IT personnel who create and maintain our Mōhio ecosystem and its tools and analytics.

MōhioExpress

Prompting practice staff to complete best practice health interventions


Prompting practice staff to complete best practice health interventions, MōhioExpress was developed in 2015 as a replacement of the MedTech Dashboard. The tool is vastly better in terms of speed and usability, and is now offered for non-clinical staff members to use.

Conveniently, the tool prompts General Practice staff to the eligibility of services for the patient sitting in front of them and identifies any gaps in the patient’s clinical and/or admin records. MōhioExpress also provides a quick way to access MōhioForms and other electronic tools.

Designed to integrate with most Practice Management Systems (PMS), MōhioExpress encourages practice staff to complete health interventions that are best-practice. Additionally it aligns to National Health Targets, or to local DHB and PHO performance measures.

MōhioForms

Direct budget management for General practices

MōhioForms launched in 2013 and was developed to support our Flexible Funding Pool. The idea behind MōhioForms is to give General Practices the ability to manage their own budgets and determine patient eligibility, directly at the point of care with the patient.

A range of ‘out-of-the-box’ electronic forms support the workflow and decisions of clinicians.

A great example of this is the Mōhio CVD and Diabetes form which is designed to calculate the patients Framingham five year cardiovascular-disease (CVD) risk with New Zealand adjustments. The integrated Mōhio Risk Projection tool demonstrates modifiable risk factors directly to patients.

A Health Management mobile app for whānau

Toi Whānau Health, in partnership with the National Hauora Coalition, is committed to providing holistic telehealth and local healthcare services that prioritise the health and well-being of our communities. We understand the importance of accessible healthcare and strive to ensure that our services are available anytime and anywhere. Through our telehealth offerings, we aim to bridge the gap in healthcare access, ensuring that individuals can receive the care they need from the comfort of their own homes. We believe in empowering our communities to take control of their health and provide comprehensive services that address their diverse needs. Together, we are working towards building healthier and stronger communities.

MōhioRecall

Better outreach to whānau

MōhioRecall is a mobile app that allows a trained, distributed and mobile workforce to process patient recalls. Patients are prioritised and assigned to authorised callers who can record actions and outcomes arising from the recall. Māori and high-needs whānau are prioritised.

Targeted services are smoking cessation, flu vaccination and hauora wellness checks and CVD criteria confirmation.

MōhioRecall provides an easy way for practices to capture and report data for System Level Measures.
